The conference will focus on applying implementation science to develop, implement, and evaluate evidence-based care delivery models that create relational continuity across inpatient and outpatient settings to address medical and social needs for patients at increased risk of hospitalization.

1. Build the foundation for a field of "comprehensive care"
2. Increase the spread of comprehensive care models
3. Create easy-to-share tools/deliverables for participants to continue to participate in dialogue and knowledge sharing throughout the course of the year following the conference
4. Build the foundation for future comprehensive care convening opportunities and community/field of practice
