The conference will draw on implementation science best practices and focus on reducing unnecessary health care utilization and costs through structuring, incentivizing, and evaluating comprehensive care model implementation.
Note: CME credit will be available and poster sessions will offer a chance to share your/your team's work.

2. Increase the spread of comprehensive care models
3. Create easy-to-share tools/deliverables for participants to continue to participate in dialogue and knowledge sharing throughout the course of the year following the conference
