Founding Membership Program
Founding members join during the program's first year at a founding-member rate and help shape the direction of the community.
Over the last several years, a community of practice in comprehensive, coordinated care has taken shape. It is grassroots and practitioner-led, focused on patients who are frequently hospitalized. CCI's Founding Membership Program gives that community a more durable home and will support investment in additional resources for the clinical practices advancing comprehensive care model implementation and evaluation. Your membership directly supports the broader comprehensive care community, enabling free resources and convenings like Comprehensive Care Grand Rounds to remain widely available.

Annual memberships are offered across four tiers:
